Your first appointment at Santa Rosa Foot creates the foundation for our relationship with you. During your first visit we allow time to get to know you and learn important background information about you, such as your general medical history. We also plan enough time to allow you to get to know your doctor.
What to expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. We recommend you take some time to review the "Our Office" page in this web site to familiarize yourself with the doctors and this office. It may help to relieve any unnecessary anxiety you may be feeling. We look forward to your first visit!
Patient Forms
You can save some time at your first visit by printing out and completing the New Patient Forms in advance of your appointment. In order to view or print these forms you will need Adobe Acrobat Reader installed. Completed forms may be FAXed to our offices at 707-573-0315 or bring them with you when you come in for your visit.
